In order to submit your organization for group health insurance the Urquhart Group must first have specific employee census information regarding your company. If you are a new Urquhart Group customer or an existing customer renewing your policy, you may need to fill out the employee census document linked below. Note that this document is a Microsoft Excel file and you will need that software program in order to use it.
Upon completing the employee census, please email the completed Excel document to firstname.lastname@example.org or print a copy and fax it to 770-426-7342.
